Almost 50 years after its introduction, perioperative antimicrobial prophylaxis is still controversial. European Association of Urology guidelines recommend the use of prophylactic antibiotic before transurethral prostate resection (TURP) and percutaneous nephrolithotomy (PCNL). [1]

The authors of this systematic review try to shed a light on this controversial issue, they do exhaustive organized search with scientifically sound methodology.

Their review reach to the conclusion reached by other reviews and meta-analysis. [2],[3] Meanwhile, this review leaves unaddressed many critical questions with regard to the use of prophylactic antibiotics in other settings, and does not provide guidance about the optimal type of antibiotic, its timing, or dosage. Findings of this review therefore underscore the critical need for more randomized controlled trials of high methodological quality.
